Please note that first time visits to the student clinic will require initial consultation with a trainee therapist. It is recommended to show up at least 10 minutes prior to treatment time to allow for completion of client history form.

During the student clinic visit I understand the following conditions for my massage treatment:

• Bookings are essential.
• Pre-payment request when make booking.
• Late arrival to your allocated appointment time will reduce your overall treatment time.
• All clients must be over the age of 16 years. Clients between 16-18 years old must be accompanied by a parent/guardian who must be present to give consent for treatment.

Pre Treatment

• Give Medical history (health condition and medication information)
• Allow the student to disclose my medical history and conditions to their supervisor
• Sign the Inform Consent document.

During Treatment

• The Supervisor may enter the treatment room for training and coaching purposes.
• Client must inform the student of any changes and discomfort during the massage treatment.

After Treatment

• I may reasonably expect to receive the general benefits of massage therapy, such as relaxation, reduction in muscle tension
• I agree to waive my claim of liability, assumption of risk and indemnity against the students, staff or Max Therapy, resulting from my experience at the Max Therapy Student clinic.
• I agree to fill out a feedback form for student continual improvement.

One hour Student Clinic treatment time allocation will include:

• Consultation, assessment and completion of client history & consent forms. (It is recommended to arrive at least 10 mins prior to your appointment time to allow for completion of forms)
• Undressing prior to treatment
• Massage treatment
• Time to dress after treatment completion.

The Student Clinic is simulated clinic environment that offers students an opportunity to practice their skills to become confident practitioners. You will find that each room is private and sectioned with a curtain for privacy. Music and lighting will be adjusted to suit the mood of the treatment style. Trainee therapists will respect your privacy by ensuring your are respectfully covered at all times and all client files will be confidential and secure.
